Dr. Daniella Duke (Coastal Dermatology, P.C.) has closed her Medical Dermatology practice
As of July 10, 2017, I have closed my medical dermatology practice, and I will no longer have appointments for medical dermatology issues. I did not reach this decision easily, as taking care of you has been an honor for my staff and me. However, recent health issues have lead to this decision.
I will continue to refill your prescriptions until November 30, 2017. Additionally, I will make your medical records available to you until November 30, 2017. No medical dermatology services or refills may be provided after that.
I would like to thank you for allowing me to be your dermatologist, and I feel lucky to have been able to serve our community during this time. Your health is a priority for us, so I want to review several items related to your future care.
Obtaining a copy of your medical records
If you desire to have us send a copy of your medical records to your new provider (or to you), then you will need to complete a written Medical Records Release form -- your medical records cannot be released without a written authorization from you.
Your written authorization should be signed by you, and it should include: your full name, address, birth date, and the fax number to send your medical records. The completed written authorization may either be faxed (or mailed) to us at the fax number (or mailing address) listed below. If we fax your medical record to your new provider, or to you, then there is no charge.

Selecting a new Provider
It is important that you make arrangements as soon as possible to select a new physician, so that you are able to have your future dermatology concerns addressed in a timely manner. Resources to assist you in choosing another dermatologist include contacting your medical health insurance plan to obtain a list of physicians in the area who are eligible to become your physician.
To locate a dermatologist in Connecticut:
You can get a list of physician names from the Connecticut State Medical Society on their website: http://directory.csms.org/search/. You should select “Dermatology”, and then choose by City, County, or statewide.
To locate a dermatologist in Rhode Island:
There are also several dermatologists nearby in the Westerly, Rhode Island area, and they can be found online at: http://www.health.ri.gov/find/doctorsall/.
